Collaborative Action Program
The Collaborative Action Program
(CAP) works in partnership with Mental Health Services to the
Elderly and the Royal District Nursing
Service to support older people with multiple and complex needs
in their community.
CAP provides a range of practical and social
and treatment options to enable older people who have a mental
illness and complex
needs to have an enhanced quality of life, and the support to
thrive in their own homes and communities.
Services include: personal care, social
support, re-engagement of clients into community activities
and
networks, respite, referral and nursing services.
Clients
Older people who are over 60 years of age and who have dementia
or mental illness or both and who demonstrate challenging behaviours.
People
eligible for this service may have difficulty accessing
mainstream programs
or are at risk of premature admission to institutionalised
care, and may have limited social connections and/or are
estranged from family members.
