Care Services For Older People

Transition Care

Care Services for Older People is able to provide support to people who are rehabilitating after a recent health episode resulting in a stay in hospital.

The aim of the program is to support and assist this person to restore to their pre-hospital condition.

This means that the person is able to continue their recovery, with therapy services, nursing oversight and practical supports, in either their own home, or in a residential setting. Referral is from the hospital, and the care plan is developed in conjunction with the medical staff.
