Take Five

Take Five seeks to ensure enriching life opportunities for people with disabilities ( usually an intellectual disability or an acquired brain injury) and their families. In particular it facilitates respite arrangements using community resources.

The impact of a disability on a person’s life can be far reaching. It can mean that the things that are usually taken for granted, such as carrying out normal daily activities or getting around the community, become very difficult. Many people require support to achieve these things.

Clients are individually assisted to access recreational activities, develop and maintain social networks, gain community living and independent living skills. The service works with a person, and where appropriate their family or carer, to determine the level of support needed and how support is best provided. Some people’s needs are best met by individual support while others may benefit from the enriching experience of group activities. Take Five ensures the quality of service by providing the very best training, support and supervision for staff.

Home and Community Care (HACC) funds Take Five to provide respite for people who meet the eligibility criteria and this service is free of charge. Individuals can also purchase support from Take Five privately, with compensation payments or through other Government funding. Current fee rates are available on request.

How we can help you

We help people in a number of ways:

  • to be involved in recreational activities

  • to develop and maintain social networks within the community

  • to gain community living skills

  • with high level personal care needs

  • with complex issues around adjusting to a recently acquired disability eg brain injury.

We work with a person, and where appropriate, with their family carers, to determine the level of support needed and how we might best design a service around their circumstances. Some people's needs are best met by individual support, while others may benefit greatly from the enriching opportunities of group activities.

Individual Support

Assistance with recreation activities

Often a person who has a disability such a s acquired brain injury or intellectual disability will need support to undertake the recreational activity of their choice. We specialise in providing both the practical support and, where indicated, support in integration.

Individual skills training

Fostering the independent living skills of our clients results in enhanced confidence, personal awareness and responsibility. These skills can be incorporated into social and recreational activities or through a more specific programme.

Personal care assistance

Assistance can be provided for tasks such as dressing, showering and other personal care needs. Staff have medication training and are credentialled for specific procedures.
