This programme works with older people living in the southern metropolitan area who are experiencing a mental illness.
It is particularly directed towards people who:
- require support to continue living in their own home
- are socially or physically isolated
- have no access or limited access to support services
- are under threat of homelessness
- require frequent hospital admissions for either physical or mental health concerns, and/or
- present frequently at hospital emergency departments
Southern Supported Recovery provides non-clinical psychosocial Mental Health rehabilitation and recovery support, working together with the client to enable them to increase their independence and quality of life, living with a mental illness.
Clients
Older people with a mental illness (over 65 years) and Aboriginal and Torres Strait Isander people over 45 years, who are living in the southern metropolitan area.
