In the process of providing services UnitingCare Wesley Adelaide may need to collect personal information from you.

Personal information is information from which an individual can be identified or whose identity could reasonably be ascertained.

Examples of Personal information include:

  • age, name, date of birth;

  • religion;

  • gender;

  • banking details;

  • identifying photographs;

  • a description of characteristics or behaviours

At the UnitingCare Wesley Adelaide we aim to promote greater openness regarding the handling of your personal and sensitive information.

As such all records and information management , including the collection of personal information, is undertaken by UnitingCare Wesley Adelaide following the guidelines set by the ten National Privacy Principles.

The Privacy Policy

All personal information relating to clients and employees gathered by UnitingCare Wesley Adelaide, will be collected, maintained, used, stored and disposed of in accordance with the Privacy Act 1989, the Privacy Act Amendment 2000, and the ten National Privacy Principles as set out by the Privacy Commissioner.

The UnitingCare Wesley Adelaide Privacy Policy outlines the ways in which UnitingCare Wesley Adelaide ensures that the Privacy Principles are understood and adhered to in daily organisational activities.

UnitingCare Wesley Adelaide’s Privacy Policy explains our position on handling personal information. The policy is available on the internet, intranet and in the UnitingCare Wesley Adelaide Policy Manual.

Privacy Officers

A Privacy Officer is responsible for ensuring compliance with the Privacy Policies and Procedures for every operation or function of UnitingCare Wesley Adelaide where personal information is collected.

Privacy Officers are required to conduct annual privacy audits on UnitingCare Wesley Adelaide’s practices in order to ensure compliance with Privacy Legislation.

Accessing your information, reporting changes, and lodging a complaint

Clients and Staff of UnitingCare Wesley Adelaide can access their personal information, request a change be made, or report any privacy breach, by either communicating in person to an UnitingCare Wesley Adelaide Staff member or via correspondence to our Senior Privacy Officer. In both cases the appropriate Privacy Officer will investigate and record the request and undertake any follow up that is required.

The Ten National Privacy Principles:

COLLECTION

Collection of personal information by UnitingCare Wesley Adelaide will be fair, lawful and non-intrusive. A person who is asked to disclose personal information, or from whom personal information is gathered verbally, will be told our organisation's name, the purpose of collection, that the person can get access to their personal information, to whom this information will be disclosed, and what happens if the person does not give the information. UnitingCare Wesley Adelaide will not collect personal information unless the information is necessary for one or more of its functions or activities.

USE AND DISCLOSURE

UnitingCare Wesley Adelaide will only use or disclose personal information for the purpose for which it was collected. Exceptions to this are covered in the UnitingCare Wesley Adelaide Privacy Policy.

If UnitingCare Wesley Adelaide uses or discloses personal information a written note of the use or disclosure will be made.

DATA QUALITY

UnitingCare Wesley Adelaide will take reasonable steps to:

- make sure that the personal information it collects, uses or discloses is accurate, complete and up-to date;

- protect the personal information it holds from misuse and from unauthorised access modification or disclosure;

- permanently de-identify personal information if it is no longer needed for any purpose (including archiving) for which the information may be used or disclosed.

DATA SECURITY

All UnitingCare Wesley Adelaide data and personal information will be classified and secured according to its level of sensitivity. classification type.

OPENNESS

UnitingCare Wesley Adelaide has set out three documents which clearly express it’s policies on the management of personal information:

a. UnitingCare Wesley Adelaide Privacy Policy

b. UnitingCare Wesley Adelaide Guidelines to the Privacy Policy

c. UnitingCare Wesley Adelaide Client privacy information brochure

These documents will be accessible to staff and clients. On request, UnitingCare Wesley Adelaide will always take reasonable steps to let a person know, what type of personal information it holds, for what purposes, and how it collects, holds, uses and discloses that information.

ACCESS AND CORRECTION

Wherever it is legally and ethically possible UnitingCare Wesley Adelaide will give access to personal information which the organisation holds about an individual. Where applicable requests will be acknowledged in writing and fulfilled in the most appropriate form within 30 days.

On occasion UnitingCare Wesley Adelaide may find it necessary to withhold information that has been requested. In such situations the individual will been given either:

a) the opportunity of using a mutually agreed intermediary to review the information; or

b) a reason for the denial of access.

If UnitingCare Wesley Adelaide charges for providing access to personal information, those charges:

a) will reflect only the costs involved in collection of the data; and

b) will not apply to lodging a request for access.

Individuals have the right to have amended, if necessary, any information that UnitingCare Wesley Adelaide holds about them.

IDENTIFIERS

When implementing it’s own identification system UnitingCare Wesley Adelaide will not adopt, use or disclose an identifier (number, letters, code) which has been assigned to a person by a Commonwealth government 'agency'. Exceptions include where such a use or disclosure is necessary to fulfil an obligation to a government agency.

ANONYMITY

There are situations where UnitingCare Wesley Adelaide may be able to give people the option of interacting anonymously, for example Lifeline Telephone Counselling. However, in some circumstances we will be unable to provide the option of anonymity.

TRANSBORDER FLOW OF DATA

UnitingCare Wesley Adelaide will only transfer personal information to a recipient in a foreign country in circumstances where the information will have appropriate protection.

SENSITIVE INFORMATION

UnitingCare Wesley Adelaide will not collect sensitive information unless the individual has consented, it is required by law - or in other special specified circumstances.
